Santana Hills Healthcare Center is a 167-bed facility with 90 skilled nursing beds and 77 subacute beds, providing compassionate, patient-centered care to support recovery and rehabilitation.

We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team. The ideal candidate will evaluate and treat communication, speech, language, cognitive-communication, and swallowing disorders to help residents improve their functional abilities and quality of life.

Employment Types: Full-Time, Part-Time, PRN, and Per Diem

Responsibilities

• Evaluate residents' speech, language, communication, cognitive-communication, and swallowing abilities.

• Develop individualized treatment plans based on assessment findings and resident needs.

• Provide appropriate speech-language therapy interventions to support functional outcomes.

• Monitor resident progress and adjust treatment plans as needed.

• Maintain accurate and timely documentation, including evaluations, progress notes, and treatment records.

• Educate residents, families, and caregivers on treatment techniques and strategies.

• Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate resident care.

• Screen residents who may benefit from speech-language therapy services.

• Follow physician orders, facility policies, safety procedures, and applicable regulations.

• Maintain required licensure, certifications, and professional credentials.

• Participate in continuing education and professional development to maintain clinical knowledge and skills.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.

• Current, active, and unrestricted California Speech-Language Pathologist license.

• Certificate of Clinical Competence (CCC-SLP) preferred, where applicable.

• Experience in a skilled nursing facility or long-term care setting preferred.

• Strong communication, interpersonal, and documentation skills.

• Ability to work independently and collaboratively with an interdisciplinary healthcare team.
