About the Position

Job Title: RN Home Health Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience License: Valid RN license Certifications: Required certifications as per home health care standards Must-Have: - 1 year of experience in Case Management Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is required. Experience in hospice, private care, or working as a Case Manager independently does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a Certified Home Health Agency will be considered relevant for the 1-year requirement. Ability to travel to cases as assigned is required. Weekend rotations are once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Admission processes and case management responsibilities must be performed in accordance with policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
