We are seeking a dedicated and compassionate Registered Nurse (RN)/Case Manager Nurse to join our growing home care team. This full-time/part time RN position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Northern Virginia communities. Responsibilities
- Conduct initial client assessments and routine reassessments in client home settings
- Conduct hospital discharge assessments in client home settings
- Develop and update individualized care plans
- Ensure compliance with care standards
- Must hold a current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia
- Must have experience conducting in-home clinical assessments and completing Medicaid DMAS Forms 97, 98, 99, and 100
- Ability to work independently
- Flexible Scheduling
- Competitive Compensation
- Paid mileage for eligible travel
