**Exciting Opportunity for a Case Manager - Elevate Your Career!** **Job Description:** - **Position**: Registered Nurse - Case Manager - **Shift**: Day 5x8-Hour (08:00 - 16:30) - **Experience Required**: Minimum of 2 years in Acute Care Case Management/Discharge Planning - **Patient Ratio Experience**: - Days: 1:25 - Weekends: 1:45 - **Charting System**: Proficiency in Epic required - **Travel Requirement**: Yes (Seasoned traveler preferred) **Key Responsibilities:** - Conduct assessments and evaluations of patients - Coordinate care and facilitate discharge planning - Engage in utilization review processes - Develop and implement care plans collaboratively - Organize and participate in family meetings - Ensure compliance with CMS and Joint Commission standards - Manage prior authorizations and DME arrangements **Patient Care Details:** - **Patient Age Groups**: - Neonates - Adolescents - Adults - Geriatrics - **Daily Census**: Manage a case load of up to 25 patients - **Interdisciplinary Support**: Collaborate with Physical Therapy and Social Services - **Patient Diagnoses**: Various medical conditions requiring acute care management **Qualifications:** - Must possess a valid CA state nursing license (Pending license accepted but must be licensed upon start) - Strong knowledge in behavioral health is not applicable - Ability to handle complex cases involving diverse needs such as homelessness, mental health history, and substance abuse - Competence in arranging DME, SNF, and ambulance transport independently **Additional Information:** - **Orientation Hours**: 40 - **Floating Expectation**: No - **Pre-Approved Time Off**: 1 Request allowed - **Weekend Requirement**: Every other weekend to meet department needs - **RTO Restrictions**: No more than 7 days **Why Catalytic Solutions (CatSol)**: At CatSol, we connect dedicated therapists with rewarding assignments across the country.
