Registered Dietitian (RD) – Home Health Visits

Per Diem | Somerset, Union & Middlesex Counties, NJ

BAYADA Home Health Care is seeking an experienced Registered Dietitian (RD) for a per diem opportunity with our Union County Visits team. This position provides one-on-one home health visits to adult and geriatric clients throughout Somerset, Union, and Middlesex Counties, NJ .

As a BAYADA Registered Dietitian, you'll be an integral member of an interdisciplinary home health team, providing individualized nutrition assessments, education, and medical nutrition therapy to help clients manage their health and safely remain in the place they call home.

Home health experience is preferred, but not required.

Responsibilities

• Provide home health visits to clients throughout designated territories.
• Complete comprehensive nutrition assessments and develop individualized nutrition care plans.
• Provide medical nutrition therapy to support disease management, preventative care, and overall health.
• Counsel clients and families on nutrition recommendations based on individual health needs and goals.
• Educate clients, caregivers, and clinical team members on nutrition and dietary management.
• Translate current evidence-based nutrition recommendations into practical strategies clients and caregivers can implement at home.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team.
• Monitor client progress and adjust nutrition recommendations as appropriate.
• Complete timely and accurate clinical documentation using BAYADA's electronic medical record (EMR) system.

Qualifications

• Bachelor's degree or higher in nutrition, dietetics, or a related field from an accredited institution.
• Current Registered Dietitian (RD/RDN) credential.
• Must meet all applicable New Jersey requirements to practice as a Registered Dietitian.
• Minimum of two years of recent experience as a Registered Dietitian in a healthcare setting.
• Home health experience preferred, but not required.
• Strong communication, education, and interpersonal skills.
• Ability to work independently and effectively manage time.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel to client homes throughout the assigned territory.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$60.00 - $65.00 per point
