Key Responsibilities

• Provide physical therapy evaluations and treatments to home health patients
• Develop and implement individualized care plans based on patient needs and goals
• Travel to patient homes to deliver therapy services (approximately 25% travel required)
• Monitor and document patient progress, adjusting treatment plans as necessary
• Collaborate with interdisciplinary care teams to ensure coordinated patient care

Required Qualifications

• Active California Physical Therapist license required
• Current Basic Life Support (BLS) certification required
• Ability to travel approximately 25% as part of regular job duties
• Graduate of an accredited Physical Therapy program
