No on-call, weekends, or holidays.Generous Paid Time Off401(k)Comprehensive Health BenefitsMalpractice insurance providedLicense reimbursement and CEU supportPaid training and onboardingOngoing education and clinical support, including physician-level consult accessAutonomy with Support to deliver independent care with access to physician-level consults.Key ResponsibilitiesDeliver person-centered care using evidence-based practicesCollaborate with interdisciplinary facility teamsConduct assessments, develop treatment plans, and manage follow-up careProvide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)Educate patients, families, and staffComplete timely documentation (within 24 hours)Compliance with facility, state and federal regulations/protocols.Qualifications:Active license as a Nurse Practitioner (NP)Minimum 2 years of post-certification experienceExperience in SNFs, hospitalist roles, emergency departments, or ICU preferredProficient with EHRs and clinical technologyStrong communicator with a collaborative mindset #J-18808-Ljbffr Impact-Health-P
