Occupational Therapist
DayToDay Health
Provide evidence-based evaluation and treatment to patients in acute, inpatient rehab, and outpatient settings. Develop individualized care plans and deliver therapeutic interventions to enhance functional independence. Educate patients and families on adaptive techniques and equipment. Collaborate with physicians, nurses, PT/SLP, and social workers as part of a team. Participate in discharge planning and document in the EMR. Support quality, safety, and performance improvement initiatives across the system.
