Certification Details BLSACLSNH or Compact RN License Job Details 12 week assignment at our main campus in Dover, NH. 4x10 shifts with a rotating day off. On call required: Monday through Friday and weekends with a minimum of 10-12 total days per month. Candidates that live less than 40 miles from the facility will not be considered. Job Requirements NH or Compact RN License required. BLS certification required. ACLS certification required. Schedule Information Shift: 07:00 - 17:30. 4x10 shifts with a rotating day off. Additional Details Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test required if titer result is not immune or low immunity. Strongly preferred Hepatitis B vaccination or proof of immunity or declination. Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or documentation of a negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break. Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. Vaccine Tdap within 10 years and updated upon expiration preferred. MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break (30 day break for Wentworth Douglas). MGB Resume including past 7 years of work experience required for submission. Two references within 4 years of submission required, minimum one supervisor or above in skillset. MGB Skills Checklist must be completed within 1 year of submission. MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions upon acceptance of assignment. If positive on a TB skin test or TB Titer, documentation of a chest x-ray indicating no active tuberculosis is required. MGB Education Verification through background check company required. MGB SSN Trace must be run with criminal check and list addresses/aliases for past 7 years. MGB National Sex Offender Check run within 30 days of start and updated annually. MGB Exclusion Checks run within 30 days of start and annually. MGB New Hampshire State Checks run within 30 days prior to start date and updated if >30 day break. MGB Name Change Documentation required if credential documentation does not match current legal name. MGB TB Symptom Survey to determine no symptoms suggestive of TB infection must be completed prior to start and updated annually.
