Community Health & Housing Navigator
Service For the Underserved
Services for the UnderServed is seeking a Community Coordinator in New York to remove barriers to care by connecting clients to housing, wellness, and stabilization services on SUS ACT Teams. You will coordinate telehealth with providers, schedule medical and mental health appointments and support transitions to housing. Ideal candidates have a HS Diploma or GED (or peer certification) and a related bachelor's degree, with experience in mental health services. #J-18808-Ljbffr Services for the UnderServed
