Contract (14 Weeks) | Days | Full-Time | Onsite
Key Responsibilities
- Provide occupational therapy services to patients across split facility locations
- Evaluate patients and develop individualized treatment plans to improve functional abilities
- Implement therapeutic interventions targeting activities of daily living, fine motor skills, and cognitive function
- Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Illinois
- Graduate of an accredited Occupational Therapy program
- NBCOT certification (or eligibility)
- Ability to work across multiple facility locations as required
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Experience working in a float or multi-site therapy role
