Must-Have:

• Experience with assessing patients, care coordination, and discharge planning.
• Knowledge of adoption/foster care, drug/alcohol abuse treatment options, and advance directives.
• Skills in crisis intervention and developing treatment plans.

Description:

Start date is ASAP. Weekend coverage required, every other weekend in rotation, 2 weekends in a 4-week period. The average daily census is up to 22-25 patients. Experience with involuntary holds and emergency department settings will be beneficial. The role includes participation in family meetings and collaborative care coordination. Candidates should possess the ability to document patient information accurately and timely. Responsibilities include developing plans of care to meet patient and caregiver needs. Candidates must be adaptable and willing to float between facilities if required. Travelers should commit to working 3 of the 5 major holidays. References required covering experience within the last 3 years. Candidates should not reach out to facility contacts directly unless for an agreed-upon interview.
