What You’ll Do Conduct home visits to assess client needs, administer prescribed medications, perform wound care, and manage medical devices. Follow individualized care plans and collaborate directly with the Nurse Case Manager before making necessary adjustments. Measure, record, and report any deviations from a client’s baseline vital signs. Identify changing health needs, advocate for increased care services or hours, and coordinate implementation with the client’s responsible party. Educate clients and families on dietary recommendations, nutritional goals, and care protocols. Communicate proactively with physicians and family members regarding health status, concerns, and clinical updates. Provide safe client transportation and advocacy for required medical appointments as needed. Complete, accurate, timely clinical documentation that reflects high professional standards. Qualifications Required License: Active, unencumbered Registered Nurse (RN) license in the State of Connecticut. Required Experience: Minimum of 3 years of home health, private duty, or community-based nursing experience. Vehicle & Travel: Valid driver’s license, reliable personal vehicle, and active auto insurance. Core Competencies: Strong independent clinical judgment, acute observational skills, and strong initiative. Physical & Background: Ability to travel throughout New London County, assist with client transfers as needed, and satisfy all state background and health onboarding requirements.
