In such cases, the agency may invoice Upstream for either the actual hours worked or up to four (4) hours of servicewhichever is greaterat the standard weekday bill rate./ppI Cancellation: If your provider cancels less than 14 days prior to starting, a fee of 12 hours x the bill rate will be assessed. If your provider terminates without 30 days notice, a fee of 36 hours x the bill rate will be assessed. These fees are passed directly to the facility per the contract terms./p/div
