Work Environment

This position is based onsite at our outpatient facility in Freehold, NJ. Key Responsibilities

• Conduct comprehensive evaluations, including review of medical history and specific assessments, to establish patient needs and develop individualized treatment plans based on findings.
• Effectively communicate evaluation results and treatment strategies to patients and their families, ensuring collaboration in setting realistic goals.
• Implement and adapt treatment protocols grounded in the latest evidence-based practices while considering each patient's specific condition and progress.
• Engage in multidisciplinary consultations with other healthcare providers as necessary, documenting any changes in patient health status that require adjustment to care plans.
• Oversee patient discharge planning and follow-up care to ensure continuity and effectiveness of treatment, with a focus on achieving measurable outcomes.
• Maintain precise documentation of all patient interactions, including evaluations, treatment plans, progress notes, and discharge summaries, in compliance with healthcare regulations.
• Demonstrate effective time management skills, meeting expected productivity levels while being punctual and organized.
• Participate actively in marketing initiatives and community outreach to enhance clinic visibility and foster a positive community relationship.
• Contribute to departmental goals and compliance with regulations, and take initiative in pursuing professional development opportunities.
• Promote superior customer service and create a welcoming environment while addressing any concerns or barriers to patient access.

Patient Population / Client Focus

This role involves working with a broad patient demographic, including individuals recovering from surgeries, injury rehabilitation, and chronic pain management. You will be responsible for developing and implementing treatment plans tailored to specific diagnoses and patient needs, ensuring that each person receives the highest standard of care.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required for this position.
• A valid DPT license in the state of New Jersey is mandatory.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is essential.
• Strong communication and interpersonal skills are important for patient interactions and collaboration with colleagues.
• Experience in outpatient therapy settings is preferred, demonstrating the ability to work independently and as part of a team.

Preferred Experience

• While two years of experience in physical therapy is desired, we value enthusiasm and dedication to continuous learning as critical attributes for success in our team.
• Previous experience using electronic health records (EHR) and documentation systems can be advantageous.
