Responsibilities
- Perform PT evaluations and provide treatment for adult residents in a long-term care setting.
- Develop, implement, and adjust individualized plans of care to meet patient goals.
- Deliver therapy modalities and interventions, including therapeutic exercise, gait training, balance activities, and pain management as appropriate.
- Document patient progress in the medical record and EMR; communicate effectively with the supervising therapist and the interprofessional team.
- Collaborate with nursing staff and other disciplines to optimize outcomes; participate in discharge planning and caregiver education.
- Educate residents and caregivers on safe home exercise programs, fall prevention, and activity modification strategies.

Requirements
- Active Physical Therapist license in South Dakota or eligibility to practice in SD.
- Current CPR/BLS certification.
- Experience with geriatric and long‑term care populations preferred.
- Proficiency in documentation and electronic medical records; strong communication skills.
- Ability to work Days (Monday–Friday) and participate in a travel assignment.
- Valid driver’s license and reliable transportation preferred.
