What We're Looking For
Trillium Health Resources has a career opening for a Residential Health Specialist to join our team! The Residential Health Specialist plays a key role in ensuring members receive the appropriate support they need within a suitable living environment. The Residential Health Specialist is responsible for identifying and connecting members with residential and other provider agencies based on their specific, individual needs.
On a Typical Day, You Might:
- Locate residential and other provider agencies for members enrolled in Tailored Care Management.
- Work closely with the Primary Care Manager and Regional Care Teams to locate appropriate residential and other supports based on member's level of care needs.
- Complete required documentation/paperwork/tasks in a software platform according to required timelines.
- Assist with integrated care management through multi-disciplinary collaborations and referrals.

Qualifications
Required:
- High School Diploma or GED and a minimum of one (1) year working experience focused on working with individuals with Intellectual Developmental Disabilities (IDD) or Traumatic Brain Injury (TBI), or BH/MH/SU, or Innovations populations; OR
- Equivalent combination of education/experience.
- Must have a valid driver's license.
- Must reside within Trillium's catchment area which includes North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
- Must be able to travel within catchment as required.
Preferred:
- A High School Diploma is preferred.
- Experience working in Long‑Term Services & Supports (LTSS)
