Job Title: Case Manager Profession: RN Specialty: Case Management Duration: 12 months Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: 2 years minimum License: Current Registered Nurse or Healthcare professional licensure Certifications: CM certification preferred; Certification in Case Management or Rehabilitation Nursing preferred Must-Have Skills/Experience: CM experience Communication skills Utilization Review experience Description: This position is for an Acute Rehabilitation Case Manager. The role is open to seasoned Case Managers with adult acute hospital experience. The hospital is a state-of-the-art, 60-bed inpatient acute rehabilitation facility dedicated to treatment and recovery from severe injury or illness. The position involves working within a team to provide comprehensive case management services. Effective oral and written communication skills in English are required. Basic computer proficiency in applications such as Excel, Word, and Outlook is necessary. Good organizational skills, time management capabilities, and analytical ability to interpret information are essential for carrying out duties independently. Collaboration and a cooperative spirit are vital, as is a desire to work as part of a team. Confidentiality principles must be recognized and adhered to rigorously. The patient population primarily consists of adults, with common diagnoses including stroke and multiple medical debilitation. Participants will have an average length of stay of about 12 days. Candidates should possess an understanding of balancing patient care advocacy with resource stewardship. Professional communication skills are needed for discussions with patients and families regarding insurance and healthcare matters that may be challenging.
