Must speak Hattian Creole. Full-time outpatient NP role. Medicare patient population. Dedicated Medical Scribe support. Reports to Center Medical Director. Provide routine and acute primary care visits. Conduct annual wellness visits and HRAs. Coordinate care with specialists and care teams. Support population health and quality initiatives. Patient education and care planning. Active NP license and DEA. 2 years of primary care outpatient experience. HCC / ICD-10 / HRA familiarity. Pay range $104,442.00 $224,952.00.
