Speech Language Pathologist - Per Diem

Evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities Participate in developing and periodically reviewing Plans of Treatment and Plans of Care Assess and treat disorders of speech, voice, language, hearing, and swallowing Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening Observe, record, and report patient condition changes and treatment responses Instruct patients on alternative communication systems when appropriate Counsel and instruct patients, families, and healthcare staff Maintain and submit required documentation Prepare written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports Participate in care coordination and discharge planning Maintain confidentiality and disclose information only when authorized and in the patient's best interests Requirements Meet education and experience requirements for ASHA Certification of Clinical Competence in Speech-Language Pathology or Audiology Minimum six months of experience as a speech therapist or speech-language pathologist Current and unrestricted license Current CPR certification Valid driver's license Auto insurance and reliable transportation Maintain vehicle insurance meeting residing-state minimum limits or specified Humana limits, whichever is higher Home Health experience preferred Willingness to complete tuberculosis screening if selected Core Competencies Demonstrates expertise in evaluating and providing speech/language pathology services, including assessment and treatment of communication disorders, while maintaining compliance with ASHA standards and documentation requirements.
