The hospital also offers world-class orthopedics, minimally invasive robotic surgery, life-saving emergency care, stroke treatment, labor & delivery, and so much more.ResponsibilitiesThe Case Management Care Coordinator assists in the clerical functions of the discharge planning process; the coordination, collection and recording of Case Management information in compliance with Medicare, Medicaid, and third party payers; ensuring discharge needs are met and that the patient and their families understand the availability of resources for their medical and social support and education.QualificationsEducation:- High School Diploma/GEDLicensure/Certification/Registration: - N/ASpecial Skills/Qualifications/Additional Training/Experience Required:- Must be able to read, write, speak and understand English.- Computer operation, organizational skills, ability to communicate well and prioritize, prior health care experience
