Enrollment Assessment Nurse

• $60-$65/hour
• Employment Type: Contract to hire
• Hours/Week: Monday-Friday, flexible between 8:00 AM-8:00 PM / 35 hours per week
• IT Equipment: Provided
• Education: Associate's degree required; BSN preferred

Position Overview

We are seeking an experienced Registered Nurse (RN) to support the enrollment of members into a managed long-term care program. This role focuses on completing comprehensive initial clinical and functional assessments, evaluating member needs, educating prospective members and families, and helping ensure individuals understand available services and next steps.

The ideal candidate will have strong assessment and interviewing skills, experience with MLTC and UAS , and the ability to explain complex healthcare information clearly and compassionately.

Required Qualifications

• Active New York State RN license .
• Associate's degree in Nursing or equivalent nursing education.
• Minimum 2 years of clinical nursing experience .
• MLTC experience required.
• UAS experience strongly preferred/required for the role.
• Strong clinical assessment and interviewing skills.
• Experience reviewing medical histories and identifying relevant clinical information.
• Understanding of ADLs, IADLs, functional status, and care needs.
• Valid New York State identification.

Preferred Qualifications

• Medicaid or Medicare experience.
• Experience with UAS or other standardized assessments.
• Home health nursing experience.
• Care management or case management experience.
• Health plan experience.
• Experience conducting intake, admission, eligibility, or enrollment assessments.
• Experience working with older adults or individuals with complex/chronic conditions.
• BSN.
• Bilingual skills.

Languages of interest: Spanish, Urdu, Bengali, Chinese/Mandarin.

Work Schedule & Location

• Schedule: Monday-Friday, flexible between 8:00 AM-8:00 PM
• Hours: 35 hours per week
• Service Area: Brooklyn, Queens, Manhattan, and the Bronx
• Candidates should be comfortable traveling within the assigned borough/service area as needed.
• The role involves approximately 3 field cases within one borough , rather than requiring regular travel across all boroughs.
