HIV Care Case Manager: Integrated Support & Coordination

Promethean Consulting Inc. seeks a Case Manager to join the HIV Clinical Care Team, manage a caseload, perform assessments, and submit encounter notes. The role covers Maryland’s 23 counties and Baltimore City. You will facilitate access to intake and screening, develop comprehensive care plans, coordinate services, monitor medications, and maintain full credentialing and confidentiality in the electronic health record system. #J-18808-Ljbffr Promethean Consulting Inc.
