Working Pattern: On-site, with Manager On-Call Rotation
Salary: $105,000 per year, commensurate with experience
We are seeking an experienced MDS Nurse Manager who is passionate about improving the health, well-being, and quality of life of older adults.
\nThis is a full-time leadership opportunity within senior care, with responsibility for overseeing MDS documentation, resident assessments, care planning, and ensuring nursing documentation accurately reflects the care provided.
\nYou'll work closely with residents, families, nursing colleagues, and the wider interdisciplinary team to ensure each resident receives coordinated, individualized, and high-quality care.

Oversee and verify documentation relating to nursing care provided to residents

Manage resident MDS data collection and documentation
\n - \n
Complete resident assessments accurately and within required timeframes
\n - \n
Develop and maintain individualized resident care plans
\n - \n
Complete required RAP summaries
\n - \n
Ensure MDS processes and documentation comply with OBRA requirements
\n - \n
Manage care plan schedules and ensure required reviews are completed
\n - \n
Meet with residents and their families to discuss plans of care
\n - \n
Collaborate with the interdisciplinary team to review and coordinate resident care
\n - \n
Maintain accurate, complete, and compliant clinical documentation
\n - \n
Support high standards of resident care, safety, and quality of life
\n - \n
Participate in the Manager On-Call Rotation

Current Registered Nurse (RN) license in good standing in Illinois
\n - \n
Minimum of 2 years of MDS experience
\n - \n
Strong understanding of resident assessment and care planning
\n - \n
Excellent clinical documentation and organizational skills
\n - \n
Strong communication and interdisciplinary teamwork skills
\n - \n
Commitment to delivering high-quality, resident-centered care

Current MDS certification

Candidates without MDS certification must be willing to obtain certification within 90 days of employment
