Registered Nurse - Ambulatory Care/Clinic/Outpatient

Job Summary:
The Tele/Ambulatory Care RN provides comprehensive nursing care in an outpatient or ambulatory setting, focusing on assessment, education, and coordination of patient care. This role ensures holistic and individualized patient management, facilitating smooth transitions and effective communication among care team members.

Responsibilities:

• Assess patient conditions and develop individualized care plans aligned with psychological, sociocultural, spiritual, and lifestyle factors.
• Deliver direct patient care, including monitoring vital signs and administering treatments within telemetry and PACU settings.
• Educate patients and their families about health conditions, treatment options, and post-discharge care plans.
• Collaborate with physicians, support staff, and interdisciplinary teams to coordinate effective patient care delivery.
• Document clinical assessments, interventions, and patient responses accurately and timely.
• Maintain compliance with licensure, certification, and hospital policies to ensure high-quality standards.

Qualifications:

• Current CA RN licensure with Nursys verification uploaded within the past 2 weeks.
• Valid AHA BLS certification required.
• Minimum of 2 years of recent telemetry and ACU/PACU nursing experience.
• Strong understanding of acute and ambulatory patient care procedures.
• Ability to work 4x10-hour shifts from 6:00 AM to 4:30 PM or 10:00 AM to 8:30 PM.
• Successful completion of onboarding modules and 7-year work history verification.
