Registered Nurse - Hospice

Job Description

Job Summary:
Provides skilled nursing care to hospice patients in accordance with physicians' orders and agency policies. Coordinates, supervises, and evaluates home health staff to ensure quality patient care and compliance with regulatory standards.

Responsibilities:

• Assess patients' needs and develop personalized care plans based on clinical judgment and nursing process.
• Administer medications, treatments, and interventions as prescribed by physicians.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses to maintain high-quality care standards.
• Document patient visits, treatments, and progress accurately in the EMR system within specified timeframes.
• Communicate regularly with the interdisciplinary team to coordinate care and update on patient status.
• Travel up to 75 miles from the care center to perform home visits and attend meetings as required.

Qualifications:

• Hold an active RN license in the state of practice.
• Minimum of 1 year of recent hospice or adult home health experience.
• Proficient in HomeCare HomeBase (HCHB) EMR; experience with Oasis is required for RNs.
• Reliable transportation, a valid driver's license, and agency-required liability insurance.
• Ability to work on call shifts and travel extensively within designated territory.
