Advanced Practice Provider | Outpatient Primary Care #26-139 Job Description We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes—not rushed visits and volume-driven metrics. As an APP, you will provide comprehensive primary care services within a patient panel model, supported by interdisciplinary teams, aiming to keep patients healthy, engaged, and out of the hospital.
