RN Case Manager

CVS Health is seeking an RN Case Manager to coordinate care for diverse patient populations across healthcare and social assistance settings. In this role, you will assess patient needs, develop and implement care plans, and collaborate with providers, payors, and community resources to support quality, affordable care. You will monitor clinical outcomes, close care gaps, and promote adherence to evidence-based guidelines.
