A growing Skilled Nursing Facility in Galt, CA is seeking a dedicated and compassionate Occupational Therapist (OT) to join the rehabilitation team. This is a full‑time weekday position offering a consistent schedule and a supportive work environment. Position Details Hours: 40 Hours per week Location: Galt, CA Schedule: Monday–Friday 8:00 am–4:30 pm Rate: $75/hr ($3,000/weekly) W-2 Traveler or $70/hr ($2,800/weekly) W-2 Local Job Description Perform occupational therapy evaluations and develop individualized treatment plans based on patient needs and physician orders Provide skilled therapy services focused on improving strength, mobility, coordination, and independence with daily living activities (ADLs) Implement therapeutic exercises, activities, and adaptive techniques to help patients regain functional abilities Educate patients and caregivers on treatment plans, safety precautions, and home exercise programs Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes Maintain accurate and timely documentation in compliance with facility, state, and Medicare guidelines Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate patient care Participate in care planning meetings and discharge planning as needed Ensure a safe and professional therapy environment while following all facility policies and infection control procedures Provide compassionate, patient‑centered care focused on improving quality of life and maximizing patient independence #J-18808-Ljbffr AvaMed Workforce
