Responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care. CONNECTING TO MISSION:All work is performed within a resident centered frame work in collaboration with other healthcare professionals, residents and their families. Functions as a role model for staff. Recognizes the value of each person as an individual. Supports and Promotes Resident/Patient strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based.
