LVN / LPN LTC/SNF

Job Summary:

Provides nursing care to pediatric and long-term adult patients within a designated unit, ensuring quality patient outcomes through skilled clinical interventions. Collaborates with healthcare team members to deliver safe, effective, and compassionate care in a fast-paced environment.

Responsibilities:

• 
  Administer medications, treatments, and therapies in accordance with physician orders and patient care plans.
  
  
• 
  Monitor patient status, including vital signs and clinical conditions, and document findings accurately in the electronic charting system.
  
  
• 
  Support patient mobility and comfort by utilizing equipment such as Hoyer lifts, feeding tubes, and trachs.
  
  
• 
  Communicate effectively with patients, families, and team members to coordinate care and address needs.
  
  
• 
  Maintain compliance with safety standards, including infection control protocols and equipment use.
  
  
• 
  Adapt care based on patient acuity and census fluctuations, ensuring appropriate patient ratios.
  
  

Qualifications:

• 
  Possess a valid NJ State License or Compact License at the time of submission.
  
  
• 
  Hold active BLS certification; PALS/ACLS preferred.
  
  
• 
  Demonstrate at least 1+ years of pediatric experience and long-term care experience with adult or pediatric patients.
  
  
• 
  Be proficient in handling trachs, feeding tubes, and other specialized equipment.
  
  
• 
  Exhibit strong clinical skills, including patient assessment and documentation accuracy.
