We provide discreet, personalized guidance - helping members make sense of diagnoses, coordinate care, and move confidently through the healthcare system. The Role We are seeking an experienced RN to serve as a Nurse Navigator in a concierge model. This role is ideal for a nurse who values professionalism, autonomy, and thoughtful, unhurried care. You will support a small, curated member panel and work closely with physician leadership to deliver a seamless, white-glove experience. What You’ll Do Serve as a trusted point of contact for members and families through regularly scheduled check-ins Guide members through diagnoses, treatment pathways, and appointments Coordinate care across specialists, hospitals, and services Work with physicians to prepare members for visits and clarify next steps afterward Provide calm, informed advocacy during complex or stressful moments Who We’re Looking For Active RN license 5+ years of clinical experience (oncology, geriatrics, ICU, med-surg, care management, or similar) Exceptional communication skills and emotional intelligence Comfortable working independently in a high-touch, concierge setting Strong knowledge of the healthcare landscape across Jupiter, Juno Beach, Tequesta, Palm Beach Gardens, North Palm Beach, and West Palm Beach Established relationships with local physicians, practice managers, or healthcare professionals Exceptional customer service and interpersonal skills Strong computer and technology skills, including proficiency with Microsoft Office/Microsoft 365 (Outlook, Word, Excel, Teams, and PowerPoint) and the ability to confidently use cloud-based file sharing, scheduling, communication, and healthcare systems as required.
