Overview

The CalAIM Case Coordinator supports the reintegration of CalAIM-eligible individuals by coordinating reentry care plans under the direction of the RN Discharge Planner. The role assists with Medi-Cal eligibility screenings and identifies individuals who qualify for CalAIM Justice-Involved services. The position coordinates communication and service delivery with managed care plans, in-reach providers, and community partners. The role manages documentation, timelines, and system entries to support timely service activation and continuity of care. The position operates under clinical oversight to ensure compliance with DHCS CalAIM requirements and operational alignment.

Responsibilities

• Work with Medi-Cal eligibility technicians and in-reach Health and Human Services staff to determine timing for activating Justice-Involved aid codes in alignment with DHCS policy and the 90-day pre-release window.
• Gather clinical and logistical information, document required consents, monitor release dates, and support warm handoff efforts to promote continuity of care.
• Enter required documentation into the EHR, track release timelines, and collaborate with Medi-Cal eligibility staff and in-reach partners to ensure timely service delivery.
• Activate Justice-Involved aid codes in the state’s JI Reentry Portal in accordance with pre-release requirements.
• Ensure continuity of care and compliance with CalAIM requirements while functioning under the clinical oversight of the RN Discharge Planner.

Qualifications

Education

• Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred

Experience

• Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred.
• Strong communication, documentation, and coordination skills, as well as a working knowledge of Medi-Cal processes and CalAIM policy requirements, are essential.

Licenses/Certifications

• Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
