Key Responsibilities

• Evaluate patient needs and establish individualized treatment plans, ensuring each plan is adaptable based on patient progress and feedback.
• Conduct one-on-one therapy sessions, focusing on quality care with a target of seeing 1–2 patients per hour.
• Utilize evidence-based practices, incorporating manual therapy techniques and therapeutic exercises as appropriate for each patient's condition.
• Document all therapy sessions meticulously in WebPT to maintain accurate patient records and progress reports.
• Collaborate with interdisciplinary teams to provide comprehensive care, ensuring alignment in treatment goals and methodologies.
• Educate patients and their families about rehabilitation processes and preventive care strategies, empowering them with the knowledge to aid their recovery.

Patient Population / Client Focus

This position caters to a diverse range of patients, including those recovering from surgeries, injuries, or chronic conditions. The focus on home care allows for tailored interventions that consider each patient's lifestyle, progress, and home environment, enhancing the effectiveness of rehabilitation efforts.

Qualifications

• A Doctorate or Master's degree in Physical Therapy from an accredited institution.
• Active California Physical Therapy license or the ability to obtain one prior to the start date.
• Proven expertise in manual therapy and clinical assessment skills.
• A commitment to patient-centered care and a passion for achieving meaningful patient outcomes.

Preferred Experience

Candidates should have a minimum of 2 years of clinical experience in physical therapy, preferably with hands-on experience in home health settings. Familiarity with chronic care management and post-surgical rehabilitation is desirable. Proficiency in digital documentation and a collaborative spirit are also highly valued.

Compensation and Benefits

The successful candidate will receive a competitive salary in the range of $120,000 to $130,000 per year, commensurate with experience. Our comprehensive benefits package includes:
