Work Environment

This full-time position offers an onsite work environment where you will engage directly with patients in their homes. Our home care facility is equipped with the necessary resources to support advanced nursing practices. As a Primary Nurse Practitioner, your daily schedule will typically involve 12-hour shifts, averaging 36 to 40 hours per week, enabling you to achieve a rewarding work-life balance while maintaining around-the-clock care availability for our patients.

Key Responsibilities

In your role as a Primary Nurse Practitioner, you will:

• Conduct detailed assessments and accurate diagnoses of patients presenting with both acute and chronic medical conditions.
• Collaborate with physicians and other healthcare professionals to design and implement evidence-based treatment plans tailored to each patient’s needs.
• Order, interpret, and analyze diagnostic tests to inform patient care decisions, including performing necessary follow-up assessments.
• Prescribe medications and therapies, ensuring adherence to local and federal regulations.
• Educate patients and their families about disease management, medication protocols, and recovery strategies, focusing on empowering them in their healthcare journey.
• Actively participate in interdisciplinary team meetings to discuss patient progress and adapt care plans as needed.
• Maintain comprehensive and accurate electronic medical records, documenting all patient interactions and treatment plans in compliance with organizational standards.

Qualifications

Compensation and Benefits
