Physician Assistant

Job Overview Signify Health, part of CVS Health®, is hiring a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In‑Home Health Evaluations. The role involves meeting patients in their homes to perform a 45‑minute assessment that includes medication and medical history review, a physical evaluation, and, when ordered, diagnostic tests such as diabetic eye exams or spirometry. The clinician uses an iPad and an intuitive workflow but is not required to prescribe medication, order lab tests, or alter existing treatment regimens. Key Responsibilities Build trusted relationships and provide personalized health evaluations in patients’ homes. Connect with thousands of individuals nationwide to assess their unique health needs. Collaborate with the Signify Health clinical team and fulfill all role‑specific duties. Travel locally and within licensed states as required. Conduct virtual visits when necessary. Deliver Diagnostic & Preventive Services, including spirometry, diabetic eye exams, bone density screening, and other indicated tests. Required & Preferred Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Qualified to work in Worcester and surrounding area. Compensation & Hours Anticipated Weekly Hours: 29.5 Time Type: Part‑time Pay Range: $46.03 – $99.14 per hour (based on experience, education, geography, and other factors).
