In this role, you will deliver evidence-based therapy to medically complex and post-surgical patients, focusing on restoring mobility, function, and independence. You’ll perform comprehensive evaluations, create and adjust treatment plans, and collaborate closely with nursing, physicians, and case management to support safe, effective discharges. You’ll document thoroughly in the EMR, educate patients and families, and contribute to quality initiatives within a collaborative, patient-centered care environment.
