Qualifications Active Florida RN license and reliable transportation 3+ years of nursing experience Preferred Home health or field‑based experience SOC/recert/discharge experience Strong documentation & OASIS Spanish a plus You start your day as a Home Care RN by mapping your visits within your chosen territory, then step into homes to complete SOCs, recerts, routine skilled visits, and discharges – performing thorough assessments, implementing the plan of care, and teaching patients and caregivers in language that sticks; between visits, you capture accurate EMR documentation (assessments, care, orders, signatures) to keep care billable and coordinated; you update and align with physicians, therapists, care coordinators, and admin staff, responding quickly to changes in condition, safety concerns, or non‑compliance to prevent avoidable ED trips; you round out your day by reviewing QA insights and ensuring policies and payer rules are met so your patients keep progressing at home.
