The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.Providence caregivers are not simply valued – they’re invaluable. PharmD in PHARMACY from an accredited School of Pharmacy.
