Physical Therapist (PT) - General

Physical Therapist

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license, reliable transportation, and insurance
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and evaluate appropriateness of care.
• Document, prepare, and review Plans of Care (POC), progress notes, and other clinical documentation as required.
• Plan and provide treatments for various disorders based on medical referrals and therapy assessments.
• Instruct patients and families in safe transfer techniques and the use of mobility equipment.
• Educate patients and families on therapeutic procedures considering age, condition, culture, and learning readiness.
• Collaborate with interdisciplinary teams and oversee team members in patient care activities.
• Apply infection control and universal precautions in patient care.
• Accept clinical assignments within the scope of education and competence.

Compliance / Notes:

• Meets mandatory continuing education requirements.
• Must be able to perform heavy lifting, bending, pulling, pushing, and prolonged standing/walking.
• Required to work under potentially stressful conditions to meet patient needs.
