Weekend Intake CoordinatorVisiting Nurses Association (VNA)Schedule: Weekends (Fridays, Saturdays, Sundays & Mondays)Pay will be analyzed and reviewed based on level of experience for each position. Position SummaryVisiting Nurses Association (VNA) is seeking a clinically knowledgeable Weekend Intake Coordinator provide coverage on Fridays, Saturdays, Sundays and Mondays. This role serves as a key administrative liaison during weekend operations, ensuring timely intake, and appropriate service coordination.The ideal candidate demonstrates strong clinical judgment, attention to detail, and the ability to assess referrals efficiently while maintaining strict confidentiality, dependable, professional and proper time management skills.Key ResponsibilitiesReview and clinically assess Home Health and Hospice referrals for appropriateness and urgencyObtain and verify referral documentation, physician orders, and insurance informationCollaborate with on-call nurses, case managers, and leadership to coordinate weekend admissionsEnter accurate referral and patient data into the electronic medical record (EMR)Communicate professionally with referral sources, patients, families, and internal clinical staffEnsure compliance with HIPAA, CMS, and state regulationsMaintain clear and timely documentation of all intake activitiesRequired QualificationsExperience in Home Health, Hospice, or clinical care setting preferredStrong understanding of clinical terminology and patient care workflowsExcellent communication and critical-thinking skillsAbility to work independently and manage time-sensitive referralsReliable weekend availability requiredVisiting Nurses Association (VNA) is an Equal Opportunity Employer.
