Description
The Community Health Worker provides care coordination and patient support following hospital discharge or field visits. The CHW does not perform in home visits. This role focuses on follow up communication, confirming medication access, ensuring appointment completion, addressing resource needs, and completing tasks assigned by the NP, EMT, or Social Worker.
Qualifications
Experience as a CHW, care coordinator, or patient engagement specialist preferred
Experience supporting patients with chronic conditions preferred
Strong communication and organizational skills
Ability to work across multiple disciplines to support patient needs
