We are seeking a dedicated and detail‑oriented Housekeeper to join our skilled nursing facility team. RESPONSIBILITIES As the Social Services Director, you will be responsible for planning, developing, organizing, implementing, evaluating, and directing social service initiatives, ensuring the medically related emotional and social needs of each resident are met and maintained on an individual basis. Lead and assist in the planning, development, organization, implementation, evaluation, and direction of the facility's social service programs. Actively participate in discharge planning, and the development and implementation of social care plans and resident assessments. Conduct interviews with residents and their families as needed. Maintain accurate and timely Social Service progress notes, documenting responses to treatment plans and adjustments to institutional life. Facilitate access to community social, health, and welfare agency resources to meet resident needs. Maintain consistent contact with residents' families providing progress reports as necessary. Assist in the effective resolution of grievances voiced by residents, families, responsible parties, or ombudsman. QUALIFICATIONS Bachelor’s in Social Worker related field (Sociology, Special Education, Rehabilitation Counseling, Psychology, etc.) Minimum of 1 year experience as a Supervisor in a Skilled Nursing Facility WHAT WE OFFER We value our team members and provide a supportive and enriching work environment.
