Preston Taylor Community Health Centers Inc, is composed of a team of dedicated health care professionals that have at heart the mission of the organization of providing comprehensive primary health care services to the residents of the four-county service area of West Virginia in Preston, Taylor, Tucker, and Grant Counties regardless of their ability to pay. This is accomplished through the provision of primary care services that are culturally sensitive and relevant to the needs of the community. 

In supporting the delivery of activities deriving from 6 Community Health Care Medical Centers, 2 Pharmacies and 2 Dental Centers, we are seeking a Mid-Level Health Center Provider (Physician Assistant and/or Advanced Practice Nurse Practitioner) to join the organization, and this position will work at the Medical Center of Taylor County and from the schools located in the Taylor County, West Virginia school district. 

Primary Responsibilities:

• Follow guidelines per HIPAA & FERPA regarding patient's PHI, OSHA, and other organizational policies regarding patient care. 
• Provides a full range of pediatric medical services from general preventive care to ongoing acute and chronic illness management.
• Deliver primary health care to patients in a school based setting and/or the health center. 
• Responsible for diagnosis and treatment of common and chronic illnesses, planning of periodic health assessment, vaccines, promoting optimal health and preventative care. 
• Provide community referrals and regulatory compliance documentation. 

Qualifications 

• Graduation from an accredited Physician Assistant or Nurse Practitioner Program. 
• Current WV license to practice. 
• Experience working with schools and public health preferred. 
• Strong computer skills and proficiency with electronic health records. 
• Strong communication skills and knowledge of the communities of which we serve. 
• Must be well organized with a strong attention to detail and accuracy
