Company: Compassus Position Summary The Nurse Practitioner – PPV is accountable for modeling Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation while promoting the Compassus philosophy based on the 6 Pillars of Success. The role upholds the Code of Ethical Conduct and fosters positive working relationships across all departments and external stakeholders. The NP may provide palliative care to patients dealing with chronic, complex, or life‑threatening conditions at varied locations. The NP performs assessments, disease and treatment education, and symptom management to meet the needs of palliative patients and their families while maintaining maximum comfort and coping. Collaboration with the PCP, specialists, and the Medical Director is crucial. Schedule flexibility and agency collaboration are required for hospice care, including routine recertification and emergency assessments. Position Specific Responsibilities If providing Hospice Care Perform routine and emergency assessments, completing recertification visits for patients on their 3rd benefit period or beyond, including admissions and transfers. Document observations, treatments, and other pertinent information. Communicate with the Medical Director, Attending Physician, DCS, and IDT regarding findings and symptom‑alleviating measures. Provide physical and emotional care during crisis periods. Prevent unwarranted hospitalizations by addressing patient and family needs at home. Outpatient Palliative Care Responsibilities (if applicable) Perform routine and emergency assessments related to outpatient palliative visits. Evaluate patients and families at the patient’s residence. Assist in diagnosing, treating, and managing acute and chronic conditions. Order, perform, and interpret laboratory and radiology tests within scope. Prescribe medications, including controlled substances within delegated authority. Order treatments and durable medical equipment as indicated. Implement therapeutic or corrective measures as needed, including urgent care. Consult palliative care physicians or designated staff as required; inform the primary physician and collaborate with other physicians. Coordinate care for palliative referrals. Maintain accurate patient records, charts, and documents to support medical practice and reimbursement. Comply with laws and regulations related to Collaborative Agreements. Initiate reimbursement for services rendered. Assist patients and families with goal setting, treatment options, and advanced care planning. Provide staff training and continuing education. Contribute to clinical practice guideline development for quality palliative care. Maintain patient confidentiality, respecting the organization’s standards. Collaborate and communicate effectively with the Medical Director, Attending Physician, DCS, and IDT for psycho‑social interventions. Complete required paperwork within 24 hours of visit completion. Educate patients and caregivers about patient care, disease process, dying process, symptom control, wound care, and goal setting. Perform additional duties as assigned. Face‑to‑Face Visits (if applicable) Conduct routine and emergency assessments. Complete recertification visits for patients on their 3rd benefit period or beyond, including admissions and transfers. Document observations, treatments, and other pertinent information. Evaluate findings with the Medical Director, Attending Physician, DCS, and IDT. Provide physical and emotional support during crisis periods. Perform additional duties as assigned. Provision of Palliative Care (if applicable) Ensure availability and communication of availability with management; minimum of 4 days of availability per month, with no guaranteed minimum visits. Triage consults by patient acuity. Provide bedside palliative care and patient/family education. Order, perform, and interpret laboratory and radiology tests as permitted. Prescribe medications, including controlled substances within delegated scope. Order treatments and durable medical equipment as indicated. Attend therapeutic measures as needed. Consult with designated palliative care physicians; inform primary physicians and collaborate with other physicians. Coordinate care for palliative referrals. Maintain accurate records and documentation for medical practice and reimbursement. Adhere to Collaborative Agreements and CMS billing guidelines for all encounters. Define professional growth goals and engage in professional activities. Engage in self‑care activities for personal and professional longevity. Maintain confidentiality of patients, families, staff, and the organization. Guide patients and families through goal identification, ongoing care plans, and IDT communication. Participate in monthly IDT meetings and market team meetings. Assist in identifying needs for additional IDT members and communicate effectively. Complete documentation within 24 hours of visit completion. Educate patients and caregivers on care, disease process, goal setting, symptom control, treatment options, prognosis, and advance care planning. Supervise nurse trainees, graduate and advanced practice nurses in field placements. Participate in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees. Perform other duties as assigned. Education and/or Experience Master’s degree in Nursing (required). Minimum of one (1) year of nursing practice or equivalent experience if providing Hospice Care. Minimum of five (5) years of nursing experience for Palliative Care; clinical experience in oncology, hospice, geriatrics, or intensive care preferred. Skills Mathematical skills: add, subtract, multiply, and divide in all units, using whole numbers, common fractions, and decimals; compute rates, ratios, and percentages. Language skills: read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations. Write reports, business correspondence, and procedure manuals. Present information effectively and respond to inquiries from leaders, team members, and external parties. Strong written and verbal communication skills. Understand, read, write, and speak English. Embrace hospice philosophy and articulate it. Strong organizational and time‑management skills. Work flexible schedule, including nights and weekends; travel as required. Maintain confidentiality of sensitive information. Certifications, Licenses, and Registrations Active, unencumbered CNS or Nurse Practitioner license in the state of employment. Certified Nurse Practitioner in Adult, Family, Acute, or Geriatric scope. Active NPI number. Maintain DEA certification if state permits prescribing controlled substances. Board Certification in Hospice and Palliative Care preferred but not required. State Specific Requirements Current collaborative physician agreement in place (all states except Iowa). Physical Demands and Work Environment Essential functions require regular standing, walking, and manual dexterity. Lift and move objects up to 50 pounds. Visual acuity: close and distance vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to bodily fluids, infectious diseases, and typical healthcare environmental conditions. Routine use of medical equipment and tools. Adaptations available for employees with disabilities.
