This role also includes a hybrid outpatient component, offering opportunities to deliver therapy services in alternative clinical or community-based settings as organizational and patient needs evolve. The hybrid structure allows flexibility while maintaining a strong focus on patient-centered care and functional outcomes.Duties and ResponsibilitiesProvides rehabilitative and skilled physical therapy services to patients in the home and, when applicable, in outpatient or community-based care settingsAssumes responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregiversDelivers patient, caregiver, and family education to support independence, safety, and continuity of careAssesses and evaluates functional status, including strength, mobility, gait, balance, ROM, and overall rehabilitation potentialDevelops and directs physical therapy treatment programs aligned with physician orders and patient goalsEvaluates environmental and functional needs to support safe participation in daily activities across care settingsPrepares and maintains accurate clinical documentation, including evaluations, progress notes, and discharge summariesCommunicates and coordinates with physicians and other healthcare professionals regarding patient progress and plan-of-care updatesInstructs caregivers and other personnel in components of the therapy program as appropriateIdentifies needs for additional services and makes referrals to other disciplines as necessaryParticipates in care conferences, discharge planning, and peer consultation activitiesMay provide oversight and supervision of Physical Therapy Assistants in accordance with organization policy and state regulationsQualifications / SkillsetsLicensed to practice as a Physical Therapist in the state of WashingtonGraduate of a CAPTE-accredited Physical Therapy program (Bachelor’s or Master’s degree)Minimum of two (2) years of experience as a Physical Therapist; home health and/or outpatient experience preferredStrong verbal, written, and organizational communication skillsCurrent CPR certificationAbility to travel within assigned service areas; must maintain a valid driver’s license, auto insurance, and reliable transportationThe employer for this position is stated in the job posting.
