Signify Health is seeking a Collaborating Physician to provide remote clinical oversight for advanced practice clinicians performing in-home evaluations for vulnerable patient populations. In this part-time role, you will review and co-sign charts, guide complex clinical decisions, and ensure adherence to evidence-based and value-based care standards. You will collaborate with interdisciplinary teams using modern telehealth and data tools to close care gaps, support risk adjustment and population health initiatives, and improve patient outcomes.
