Responsibilities

• Provide direct patient care to residents in a long-term care setting under supervision of a registered nurse
• Administer prescribed medications and document administration in the charting system
• Monitor vital signs, observe for changes, and report findings to the supervising nurse
• Assist residents with activities of daily living and hygiene, respecting dignity and independence
• Maintain accurate documentation in the electronic health record and care plans
• Collaborate with an interdisciplinary team to implement care plans and safety protocols

Requirements

• Active North Carolina LPN license or compact state license; BLS certification required
• 1+ year of geriatric long-term care/LPN experience
• PCC experience preferred
• Ability to work 8-hour night shifts (11:00 p.m.–7:00 a.m.)
