Physical Therapist (PT) – Home Health

Location: Mansfield, CT
Assignment Duration: July 20, 2026 – October 17, 2026 (12 Weeks)

Position Overview

We are seeking an experienced Physical Therapist (PT) to provide high-quality home health services to patients throughout the East Region of Connecticut , with a primary focus on the shoreline area. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in patients' homes to improve mobility, functional independence, and overall quality of life.

Key Responsibilities

• Perform comprehensive physical therapy evaluations in the home setting.
• Develop, implement, and modify individualized treatment plans based on patient needs.
• Provide skilled therapeutic interventions to improve strength, mobility, balance, and functional independence.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and injury prevention.
• Collaborate with physicians, nurses, case managers, and the interdisciplinary healthcare team.
• Maintain accurate, timely, and compliant clinical documentation.
• Travel throughout the assigned service territory to provide patient care.
  
  

Minimum Qualifications

• Active Connecticut Physical Therapist (PT) License (required prior to start date).
• Minimum 1 year of recent Home Health Physical Therapy experience .
• Valid driver's license.
• Reliable transportation.
• Current automobile insurance.
• Acceptable Motor Vehicle Report (MVR).

Pre-Employment Requirements

• Active Connecticut PT license.
• Fingerprinting required.
• Fingerprint consent form and required documentation must be submitted within 2 business days of offer acceptance.
• Valid driver's license.
• Motor Vehicle Report (MVR).
• Proof of automobile insurance.

Assignment Details

• Contract Length: 12 weeks
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• Call Requirement: None
• Coverage Area: East Region of Connecticut, with a primary focus on the shoreline area.
