Clinical Team Assistant
Visiting Nurse & Hospice of Litchfield County
Scheduler: Home Health & Hospice Agency
Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable, compassionate full-time Scheduler , preferably with home health scheduling experience to join our Home Health & Hospice team. The Scheduler plays an essential role in coordinating patient visits and supporting our clinical staff to ensure patients receive timely, consistent care.
Responsibilities
- Coordinate daily schedules for nurses, aides, therapists, and other clinical staff based on patient needs, staff availability, and geographic location.
- Schedule admissions, routine visits, and follow-up care.
- Communicate schedule changes with patients, families, clinicians, and leadership.
- Monitor schedules throughout the day and adjust as patient needs and staffing change.
- Assist with weekend, holiday, and on-call coverage.
- Maintain accurate scheduling information in the electronic medical record (EMR).
- Track missed, cancelled, and rescheduled visits.
- Assist with entering referrals and coordinating onboarding of new patients.
- Provide professional and compassionate customer service to patients, families, and staff.
Qualifications
- Healthcare, home care, hospice, medical office, or scheduling experience preferred.
- Excellent organizational, communication, and time-management skills.
- Strong attention to detail and ability to manage multiple priorities.
- Comfortable working in a fast-paced environment where schedules can change quickly.
- Strong computer skills and ability to learn new systems.
- Ability to work independently while being a collaborative team member.
Full-time, Monday through Friday position offering a competitive salary and comprehensive benefit package. Coverage for weekend & holidays needed.
